We also offer assistive technology evaluations, lending services, and advocacy to support full community inclusion.


Role Description

Under the Director of Residential Services, the Case Manager plays a key role in coordinating and overseeing in-home support services for individuals with intellectual and developmental disabilities (IDD). Provides person-centered care for a caseload of individuals, ensuring compliance with program requirements, facilitates budgeting and benefit applications, monitors health and medication needs, and conducts quality assurance checks to promote individual safety and independence. Coordinates and collaborates with families, direct support staff, and community resources.


This is an IN-PERSON, full-time, 40-hour, Monday - Friday position with core hours between 8:00am-5:00pm.

You will be working with our clients in THIER home and other community-based settings. You will be driving your personal vehicle to/from the Groton office to meet up with the clients.


ESSENTIAL FUNCTIONS

Service Coordination & Advocacy:

  • Manage a caseload of individuals in the In-Home Supports program, ensuring services align with their needs, goals, and state regulations. Oversee the implementation of individualized plans (IPs) with guidance from the Program Director. Advocate for individuals' rights, access to services, and community inclusion. Facilitate and document biannual team meetings, collaborating with medical professionals, therapists, and community organizations. Provide training and support to Direct Support Professionals (DSPs) to maintain high-quality care, reporting any performance concerns to the Program Director. Communicate with the Program Nurse as needed and participate in staff meetings.

Health & Safety Oversight:

  • Support individuals in community outings to promote independence and personal growth. Coordinate medical care by scheduling and attending appointments, ensuring transportation, and managing medication accuracy through regular checks and updates. Collaborate with the Program Nurse or Director on medical concerns.

Financial & Benefit Oversight:

  • Assist individuals with budgeting, financial tracking, and rent subsidy documentation. Ensure compliance with financial requirements, including renter’s insurance and subsidy inspections. Support applications for energy assistance, holiday programs, and annual benefit redeterminations for Medical Assistance, SNAP, and other services.

Monitoring, Compliance & Documentation:

  • Conduct home visits to assess service quality, living conditions, and equipment functionality while ensuring individuals have necessary supplies. Coordinate maintenance needs, report concerns, and ensure compliance with regulations and agency policies. Maintain accurate documentation, submit weekly reports, and communicate key updates to the Program Director. Uphold confidentiality standards, oversee residential records, respond to on-call needs, and conduct quarterly evacuation drills.


QUALIFICATIONS, EXPERIENCE & OTHER REQUIREMENTS

  • Prior experience providing direct care to individuals with intellectual and developmental disabilities (IDD), required.
  • Prior experience working with individuals with (IDD) in a case management, advocacy, or support coordination role, preferred.
  • Knowledge of Medicaid waivers, individualized service plans (ISPs), and person-centered planning, preferred.
  • Experience collaborating with interdisciplinary teams, including medical professionals, therapists, and direct support staff.

  • Ability to pass required background checks as mandated by state regulations.
  • Bachelor's degree in Social Work, Human Services, or a related field preferred
