JOB SUMMARY : Provide operational and case management to assigned residential locations.
EDUCATION OR TRAINING REQUIREMENT : Bachelor’s Degree in Social Service such as Psychology, Social Work, Sociology or related field. Must meet GA Developmental Disability Professional (DDP) requirements.
EXPERIENCE REQUIREMENT : 5 years of related experience and 5 years of supervisory experience
SPECIAL LICENSES, REGISTRATION, CERTIFICATION, OR SKILLS : GA Driver’s License and vehicle insurance; must be proficient with Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)
SUPERVISORY RESPONSIBILITY : Supervises Community living managers
ESSENTIAL JOB DUTIES ANDRESPONSIBILITIES :
Leadership
Ensures Area operates in compliance with local, state and federal rules and regulations and ensure all Community Living Arrangements (CLA’s) comply with Healthcare Facility Regulations (HFR) rules as well as Department of Behavioral Health and Developmental Disabilities (DBHDD) standards. Ensures residential locations operate in compliance with UCP policies and procedures.
- Ensure staff are compliant with all personnel and training requirements, which includes reviewing weekly relias reports and handling all emergency situations
- Work Collaboratively with HR with identifying talent for Region and ensure appropriate hiring packets are submitted by established deadlines.
- Support managers in the development of their teams.
- Schedule and facilitate monthly house meetings accomplishments. Ensure staff is focused and well trained on ISP goals and personal outcome measures
- Provide supervision to Community Living Managers (CLM’s)regularly. Conduct annual professional development and evaluations of CLM’s.
- Coordinate activities with external agencies; Support Coordinators, DFCS, Social Security, doctor’s offices, community businesses and others, representing the individual you serve
- Provides 1 : 1 training / support to identified staff with performance concerns and follow up as needed on progressive discipline.
- Approves status change and all other related HR forms related to processing of new employees
- Manages and creates on-call rotation
- Responds to all calls within 15 minutes unless not on call and responds to all emails within 24 hours.
- Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.
