The Chaplain is responsible for providing a program of spiritual care and worship for residents and the community at assigned United Church Homes (UCH) communities; and providing public and church relations activities in support of the community's goals. The Chaplain models pastoral competencies in accordance with United Church of Christ and The Association of Clinical Pastoral Education professional standards.
Essential Functions Statement(s)
- Manages the pastoral care needs for assigned community
- Provides religious guidance to residents, resident's family members and staff with respect to each person's beliefs
- Prepares, conducts and / or arranges worship opportunities to accommodate the diverse spiritual beliefs and needs of residents. This time may include sermon, bulletin and educational preparation
- Conducts timely spiritual assessments of residents upon admission, and periodically thereafter, to ascertain needs and interventions as necessary
- Participates in Interdisciplinary Patient Care Committees
- Provides support and counsel to residents' families and loved ones especially during crisis, illness and death
- When requested, acts as a liaison between staff and resident, a resident's family, or a resident's guardian during a crisis (i.e., critical illness, room transfer, change in treatment plan, etc.)
- Arranges for other ordained ministers to administer the rites and sacraments of the UCC, if requested
- Visits residents who are hospitalized or placed in other health care settings to provide support and address any spiritual or religious needs
- Meets with new residents to orient them to the pastoral care program
- Actively participates in the Interdisciplinary Patient Care Committees and other committees as assigned
- Conducts, assists and / or arranges funeral or memorial services, as requested and at the discretion of family
- Makes condolence calls as a representative of the community
- Makes appropriate referrals for psychiatric, legal, social, or other professional counsel to the appropriate team member
- Arranges for other special religious services (i.e., baptisms, weddings, etc)
- Plans, conducts, and / or arranges study groups, Bible study, prayer groups, Lenten reflections, etc.
- Develops programs to actively encourage outside clergy to visit residents
- Keeps chapel and other religious / related areas in good order, requesting maintenance as needed; maintains chapel schedule
- Assists with transporting residents to and from worship services, as appropriate
- Organizes the office and work schedule to effectively and efficiently meet the program objectives of pastoral care and visitation
- Prepare, invite others and conduct a memorial service at least annually
- Works collaboratively with social workers, physicians, nursing staff, and others, as needed
- Schedules volunteers to lead worship services on the campus as needed
- Assists marketing and admissions with providing tours or other information upon request
- Assists in representing UCH at community and church events through speaking engagements and business and health care promotion events
- Maintains communication and public relations with area hospitals, service agencies, pastors and others in the community at large
- Assists UCH with scheduled appeals and visits to donors and congregations as needed or requested
- Serves as a member of the UCH chaplains' group and attends meetings as scheduled
- Ensures department / personnel adhere to operating policies and procedures and any relevant regulations
- Organizes the work and activities of the department to achieve established objectives
- Communicates with other directors / managers and staff to integrate goals and activities
- Provides periodic reports to the Executive Director / Administrator, President and CEO and other individuals and groups as required
- Develops and annually updates departmental policy and procedure manual, and department objectives, reviewing same with any department staff
- Submits an annual plan of service and budget for approval
- Ensures the privacy, dignity, and well-being of all residents in a safe, secure environment
- Attends and participates in staff, departmental and other community meetings and sits on required committees
- Conducts departmental staff meetings (if applicable)
- Maintains awareness of individual / departmental responsibilities under established Fire Safety / Disaster plan
- Ensures the confidentiality of all resident and / or employee related information
- Ensures compliance with the established quality improvement program
- Attends all mandatory in-service training programs and required meetings
- Conducts in-service training programs as necessary
- Participates in required training from judicatory or denomination regarding Boundary Awareness Training
- Participates in performance improvement studies, committees, focus groups, corporate-wide projects, etc.
- Serves as ex-officio member to Resident Council, as appropriate
- Serves as a member of the Community's Ethics Committee
- Coordinates work of department with work of other departments
- Maintains required records and reports as outlined in UCH's Policies and Procedures manuals, including keeping track of those to be remembered at the memorial service
- Handles unusual occurrences calmly and logically to maintain continuity of business and duties
- Attend association and conference meetings
- Performs all other duties as assigned or directed
