What are the responsibilities and job description for the Navigation Services Specialist position at United Community Action Network?
Navigation Services Specialist serves as the initial contact with potential guests seeking shelter and Navigation Center services. This position is responsible for the eligibility and intake processes including, intake assessments, waitlist updates, bed management, exit strategies, data requirement, while remaining in compliance with funding requirements and standards as well as Agency visions and goals.
ESSENTIAL FUNCTIONS OF THE POSITION
The essential functions of this position requires prioritizing and completing all assigned tasks in a timely and efficient manner, adjusting for changing priorities and availability of resources, and demonstrating initiative in identifying additional job-related tasks to be completed when time permits. These duties are a representative example of position expectations. Actual duties assigned may vary and change depending on the business needs of the department and the agency.
- Complete intake and assessments on all new or potential shelter guests. Explain program, gather and verify information given and determine needed services or accommodations, following program guidelines and procedures. Assist applicant in completion of Navigation Center application, process paperwork in a timely manner and discuss the shelters rules and expectations with potential and current guests.
- Monitors status of bed availability, assigns beds and works with appropriate staff to ensure beds and pods are ready for guests prior to intake.
- Assigns guests to case management, and directs to appropriate support staff for supportive services.
- Assists guests and staff with obtaining supportive services to eliminate barriers to sustainable housing opportunities, assisting with needed paperwork and appointments.
- Responsible for keeping records of incoming guests and exits, on required logs and HMIS systems, maintaining client file confidentiality.
- Assist with planning, purchasing and distributing of meals and supplies for shelter guests.
- Maintains Community Partner scheduling and attendance regularly.
