JOB STATUS: Full time (40 hours), 12 months per year
LOCATION: Willmar, MN
RESPONSIBILITIES:
The Community Connector is responsible for outreach, education, screening and enrollment of participants into many different programs and services within UCAP and the community. Community Connectors are an active participant within the community, helping to arrange a variety of program activities and events. They will conduct presentations and set-up events to increase public awareness regarding UCAP services and programs. There is a heavy emphasis on achieving Health Equity within Refugee/Immigrant/Migrant communities by nurturing and amplifying partnerships with local health care providers including Human Services and Public Health.
EXAMPLES OF DUTIES:
- Orient participants to programing across the Community and Family Services Department (CFS).
- Support, establish, and maintain community collaborations, connections and public awareness and outreach.
- Provide technical assistance and mentoring to other programs and departments.
- Assure all necessary documentation is completed in a timely and accurate manner.
- Provide health care enrollment, assessment, and program services participants
- Attend conferences, meetings, and training as needed or requested.
- Support and Model United’s Behavioral Competencies.
EDUCATION AND EXPERIENCE:
- BA degree in Human Services or related field and two (2) years' experience in a human service field or have obtained Health Services Worker Certification or a combination of five (5) years education/experience in a related field.

- Demonstrated skills in problem-solving and conflict resolution strategies.
- Demonstrated verbal and written communication skills
- Demonstrated effective interpersonal skills, and the ability to work independently and as a part of a team.
- Awareness of and sensitivity toward cultural and economic issues.
- knowledge and understanding of family systems, abuse and family violence and child development.
- Ability to travel regularly within the county and occasionally outside of the county service area.
- Bilingual proficiency in written, oral, and literarily.
- Must have a satisfactory background check.
- Reliable, regular attendance.
