Description
UCFS is hiring! We are seeking a full-time (40hr) Mobile Crisis Case Manager. This position is responsible for serving as an advocate and liaison on behalf of children and families during their episode of care with Mobile Crisis Intervention Services. Furthermore, this position is expected to provide a personable and helpful contact for families either telephonically or in person to assist in referrals and coordination of treatment.
- Support clinical staff by attending crisis evaluations, assessing for case management needs of the family and completing necessary program documentation.
- Provide advocacy services for children and their families to help stabilize and navigate current Mobile Crisis episode of care.
- Facilitate appropriate case management referrals for the family.
- Maintain documentation standards for all records required by agency licensees and payer sources such as DCF, DMHAS, CT-BHP, and Medicaid within a timely fashion.
- Participate in supervision and maintain open lines of communication with supervisor and support staff, and coordinate treatment with psychiatrists.
- Coordinate services with relevant providers involved in a given case within and outside of UCFS (e.g. school personnel, parents, police departments, DCF, courts, etc.) and support continued collaboration with other providers.

Requirements
- Bachelor degree in Human Services (BA or BS)
- A valid divers license is required
- Reliable transportation for on the job travel
- CPR certification within 4 weeks of date of hire
