Qualifications
OBJECTIVE/PURPOSE:
The Therapeutic Case Manager provides direct services to children. Allows easy access, timely response, and flexibility to deliver skill building services in the community.
MAJOR RESPONSIBILITIES:
DIRECT SERVICES: The Therapeutic Case Manager Provides access to community services and activities to children with the intent of teaching, practicing, and mastering skills to decrease difficulty and increase success in the community. Services are to be provided in groups or individually.
CLINICAL RECORD KEEPING: The Therapeutic Case Manager Initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to consumers – including, but not limited to, treatment plans, progress notes, CANS, and time sheets. Follows all UCS record-keeping procedures and performs other duties as necessary. The Case Manager is responsible for the chart management of clients that are receiving therapeutic, not clinical, services through UCS.
ADMINISTRATIVE: Attends all UCS and community meetings necessary to promote coordination and communication.
REQUIRED QUALIFICATIONS:
High School diploma or equivalent with experience providing supportive services to children.
