Supports the appropriate departments / services in managing the timely and appropriate use of resources by facilitating patient transitions across the health care continuum. Main support in planning discharges for patients evaluated by the Comprehensive Psychiatric Evaluation Program.

Minimum Required:
- Associates in Applied Science degree in related field or five (5) years experience in acute/tertiary facility and/or community health nursing.
- Bachelor’s of Science degree in health care or related field.
