Responsibilities
- Provide comprehensive wound, ostomy, and continence care to patients in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Maintain accurate and timely documentation of patient progress and care provided
Requirements
- Bachelor's degree in Nursing
- Minimum 2 years of clinical nursing experience in an acute care setting
- Completion of an accredited WOCN program within 1 year
- Current New York State Registered Nurse License
- Certification through the WOCN Certification Board within 1 year
