UNITED HOME CARE SERVICES is seeking a Home Care Nurse to provide care in the Cincinnati, OH, area.
JOB SUMMARY:
As a Home Health Nurse (RN/LPN) you will be called to care when you’re needed. Nursing care to residents on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Nursing Supervisor.
1. Provides services in accordance with the plan of care.
2. Makes the initial evaluation visit and regularly reevaluates the resident’s nursing needs.
3. Initiates the plan of care and necessary revisions.
4. Prepares clinical and progress notes for each resident visit and summaries of care conferences on his/her residents in a timely manner as per Agency policy.
5. Coordinates services
Qualifications:
As a Registered Nurse/Licensed Practical/Vocational Nurse (RN/LPN).
Licenses: RN License LPN/LVN
Certifications: CPR Certification/ BLS Certification
Education: Associate's degree
