We are seeking a GROWTH-ORIENTED DODD Program Manager/Case Manager DODD (Department of Developmental Disabilities) Specialist to join our non-medical home care team. In this role, you will be responsible for growing our base of individuals served and managing the support, and ongoing assessments for individuals with developmental disabilities already receiving our care services.
The ideal candidate will have established relationships with one or more county DODD agencies (Franklin, Delaware, Licking, Fairfield, Hamilton, Butler, Warren). This candidate will conduct initial assessments, create customized care plans, and manage follow-up visits to ensure a high quality of care and compliance with DODD standards. Hours are typically Monday through Friday from 830a-430p with some visits on nights and weekends as needed.
Key Responsibilities of DODD Program Manager/Case Manager :
- Proven experience in program development and management within the DODD, behavioral health or social work fields is required.
- Maintain and further cultivate relationships at the area county agency SSA’s.
- Conduct initial in-home assessments for new clients to evaluate their care needs and preferences.
- Develop, implement, and maintain individualized care plans in accordance with DODD requirements.
- Conduct regular follow-up assessments and adjust care plans as needed to ensure clients receive optimal support.
- Coordinate with caregivers and other staff to ensure proper implementation of care plans.
- Care Coordinator/Case Manager will serve as a point of contact for clients, families, and support teams, addressing any concerns or changes in care requirements.
- Monitor compliance with DODD standards, policies, and procedures, ensuring all documentation is accurate and up to date.
- Educate clients and families on available resources and support services.
Qualifications of Care Coordinator/Case Manager:
- Five years’ experience in, developmental disability services must be in a supervisory capacity.
- Have existing relationships within area county DODD agencies.
- Bachelor’s degree in Social Work, Nursing, Psychology, or related field preferred, or equivalent experience in a similar role.
- Experience working with individuals with developmental disabilities and familiarity with DODD policies.
- Care Coordinator/Case Manager must have strong assessment, planning, and organizational skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and make decisions in fast-paced situations.
- Proficiency in computer software for documentation and case management.
- Valid driver’s license and reliable transportation for travel to client locations.
- Must pass a criminal background check and a pre-employment drug screen;
- Must complete 40 hours of in-service training on an annual basis;
Working Hours:
- Monday through Friday with flexible hours to accommodate rare instances of night and weekend visits as needed.
Job Type: Full-time
Pay: $55,000.00 - $70,000.00 per hour
