**Job Title: Home Health Aide/CNA**
**Job Summary:**
United home health is seeking a compassionate and dedicated Home Health Aide/CNA to join our team. As a Home Health Aide, you will be responsible for providing essential care and support to patients in their homes. Your primary focus will be on assisting with daily living activities, monitoring vital signs, and ensuring the overall well-being of our patients.
**Duties:**
- Provide assistance with activities of daily living, including bathing, dressing, and grooming
- Monitor and record vital signs to ensure the health and safety of patients
- Assist with light housekeeping tasks to maintain a clean and safe environment
- Support patients with mobility issues and assist with transfers and lifts as needed
- Collaborate with healthcare professionals to ensure coordinated and effective patient care
- Maintain accurate medical records and documentation of patient interactions
- Provide emotional support and companionship to patients in need
**Skills:**
- Experience in assisted living, Home health or hospice care settings preferred
- Proficient in taking vital signs and understanding basic medical terminology
- Ability to maintain cleanliness and organization in a home care setting
- Knowledge of memory care techniques for patients with cognitive impairments
- Familiarity with medical records systems and documentation procedures
- Understanding of nursing home protocols and patient care standards
- Ability to lift and assist patients with mobility challenges
