What are the responsibilities and job description for the Home Health Aide - HHA-OVERNIGHT/WEEKENDS position at United Hospice, Inc.?
The rate for this position is $18.10/hr. This is a per diem weekend position/overnight and is not benefits eligible except as required.
Qualifications:
Certified as a NYS home health aide and maintain certification by annually completing 12 hours of approved continuing education
Working knowledge of basic first aid
Basic understanding and commitment to the hospice philosophy of care
Ability to work as a team member and use supervision effectively
Ability to maintain a high level of confidentiality concerning patient and family information
Ability to relate to a diverse population and workforce in a personable and effective manner
Ability to understand verbal and written directions
Ability to communicate effectively verbally with patients, families and staff; able to write and document as needed
Must possess computer and email skills; will require data entry into the EMR system
Physical Requirements:
Physical agility with repetitive occurrence including but not limited to: walking, running, bending, stooping, climbing stairs, kneeling, crouching, reaching, sitting and twisting
Lifting greater than 50lbs repeatedly throughout the work day
Responsibilities of HHA will include, though not be limited to:
Follows Plan of Care established by hospice nurse
Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
Performs health care procedures monitored by hospice nurse (example: reinforce clean dressings, use of bed pan, check urinary catheter, etc.)
Provides for the physical care of the patient, as stated in the HHA care plan, (example: bathing, toileting, skin care, etc.)
Assists the patient with ambulation, transfers and turning and positioning as needed
Turns and positions patients who are unable to assist
Under the guidance and instruction on primary nurse, P.T, O.T., maintains a safe environment for patients and proper use of equipment (i.e. wheelchair, Hoyer lift)
Helps patient in performing exercises prescribed by appropriate professional personnel
May plan and prepare meals for patient, feeds patient when necessary
Performs a variety of light housekeeping tasks when necessary which are essential to the patients' health care
Performs washing of patients' clothes as needed
Attends team meetings as needed
Attends twelve (12) hours of documented training annually that may come from any acceptable source
