Our service delivery care model focuses on providing patient centered care, clinical excellence and supporting patients and their families at an end-of-life illness. We are currently looking for a Hospice RN to join the United Hospice team.
This is a F ull-time Position. The salary range for this position is $74k-$105k annually and is commensurate with education and applicable experience and certifications. All full-time / benefits-eligible positions offer a generous benefits package including health insurance, 401k with profit sharing, paid time off, and other additional perks.
To be an RN Hospice Nurse Case Manager you need to be :
- A Registered RN, licensed to practice nursing in New York State.
- One to two years of recent bedside nursing experience with good clinical skills.
- Dedicated to the philosophy of hospice care.
- Believe that excellent patient care is the only acceptable standard.
- Comfortable providing community-based care.
- Some experience caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy of care.
- Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs.
- Ability to relate to a diverse population and workforce in a personable and effective manner.
- Love to work on a team of dedicated professionals from multiple disciplines.
- Must have access to a car, valid driver's license, and current insurance.
