ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
- Complete an initial, comprehensive, and ongoing assessment of patients and families to determine hospice needs.
- Record a complete physical assessment and history of current and previous illness(es).
- Provide professional nursing care by utilizing all elements of the nursing process.
- Assesses and evaluates patients’ status by:
- Writing and initiating a plan of care.
- Regularly re-evaluating patients and families/caregivers needs.
- Participating in revising the plan of care, as necessary.
- Initiate the plan of care and make necessary revisions as patients’ status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions.
- Include the patients and the families/caregivers in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician in the physician's plan of care.
- Counsel the patients and families/caregivers in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patients with the activities of daily living and for optional comfort care.
- Act as Case Manager when assigned by Clinical Director and assume responsibility to coordinate patient care for assigned caseload.
Communication
- Complete, maintain and submit accurate and relevant clinical notes regarding patients’ condition and care given.
- Record pain, symptom management changes, and outcomes as appropriate.
- Communicate with the physician regarding the patients’ needs and reports changes in the patients’ condition
- Obtain physicians' orders as required.
- Communicate with community health related persons to coordinate the care plan.
- Teach the patients and families/caregivers self-care techniques as appropriate.
- Provide medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
- Provide and maintain a safe environment for the patients.
- Assist the patients and families/caregivers and other team members in providing continuity of care.
- Work in cooperation with the families/caregivers and hospice interdisciplinary group to meet the emotional needs of the patients and families/caregivers.
Additional Duties
- Participate in on-call duties as defined by the on-call policy.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Assume responsibility for personal growth and development regarding professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfill the obligation of requested and/or accepted case assignments.
- Participate in quality assessment performance improvement activities.
- Attend required company meetings.
- Complete necessary training per licensure and the company.
- Other duties as assigned.
KNOWLEDGE AND ABILITIES
- Must have knowledge of disease processes, including end of life concerns.
- Must operate with cultural sensitivity with respect to working with people and groups of diverse, racial, ethnic, economic, and social backgrounds.
- General computer skills required.
- Experience with EMR systems preferred.

POSITION QUALIFICATIONS
- Active Registered Nurse (RN) License in the state of Arizona
- CPR Certification
- Fingerprint Card
- Must be a licensed driver with an automobile that is insured and in accordance with the state/and or organization requirements and is in good working order.
- Proven experience in a healthcare setting, preferably in hospice or palliative care.
- Strong knowledge of Quality Assessment and Performance Improvement (QAPI) principles and practices.
- Experience in developing and delivering educational programs.
- Excellent organizational and time management skills.
- Effective communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Proficiency in using computer software and electronic health records.
WORK ENVIRONMENT
- Prolonged or considerable walking or standing.
- Able to lift, position, and/or transfer patients.
- Able to lift supplies and operate equipment, as necessary.
- Considerable reaching, stooping, bending, kneeling, and/or crouching.
- Visual acuity and hearing to perform required nursing skills.
- Skilled Nursing Facilities (SNFs)/Group Homes and personal residence.
- Considerable driving and in/out of vehicle.
- Possible exposure to infectious diseases.
Job Type: Full-time
Pay: $85,000.00 - $90,000.00 per year
