What are the responsibilities and job description for the Patient Navigator position at United Hospice?
Sorry, this is not a remote position. Candidate must be able to report to our New City, NY. office in Rockland County.
United Hospice is recruiting for full-time and part-time patient navigators to work within our Intake Department to manage patient intake calls and referrals.
Qualifications :
Registered Nurse - preferred, Licensed Practical Nurse and non-nursing backgrounds will be considered.
Understanding of hospice philosophy and how hospice can meet the needs of terminally ill patients and their families
Basic understanding of hospice insurance benefits
Ability to understand the needs of terminally ill patients and their families and communicate to them how hospice can help meet those needs
Basic knowledge of medications, treatments, and therapies for pain and symptom control
Ability to communicate effectively orally and in writing
Strong computer skills
Ability to relate to a diverse population and workforce in a personable and effective manner
Good listening skills
Ability to work within a team concept
Able to follow and complete detailed workflows
Responsibilities :
Screens and processes referrals and inquiries to our hospice program promptly
Consults with patient / family to provide emotional support and to assess their physical, psychosocial, and spiritual needs
Educates the patient / family and various referral sources about hospice care and the services available
Conducts and documents insurance search with the assistance of the finance department
Refers patient information to the medical director to ascertain medical appropriateness
Records all activity on the intake form throughout the referral process
Ability to input demographic information into the computer on each referral and input updates as status changes
Schedules assessments for admission with patient / family
Responds to requests to provide information about community services for non-hospice referrals
Participates in on-call and weekend rotation
Refers to other health care providers if the patient is not hospice-appropriate or declines hospice services
Mails and logs physician packets to new physicians
Accepts other responsibilities as assigned
