We are currently seeking a Social Worker (LMSW) to join our team in Orange County. This is an in-person position working full-time out of our Goshen, NY Office. This is a full-time position; the salary range for this position is $60k - $83k annually and is commensurate with education, experience and certifications. This position is benefits eligible and includes medical, dental and vision insurance, 401(k) retirement plan with profit sharing, paid time off and other additional perks. Sign-on bonus provided to eligible candidates. Salary enhancement is provided to candidates with applicable prior hospice experience. Work-related mileage is reimbursed.
Location Primarily focused on Orange County, New York, but will be called upon, from time to time, to provide care for patients and their families in Rockland County, NY
Schedule : Day Shift (includes a weekend rotation on a monthly basis)
Job Description Summary :
Responsibilities of Social Worker & Hospital Liaison will include, though not be limited to the following :
- Must develop and maintain positive relationships with referral sources and promote exemplary customer service to physicians, facility staff, patients, and families
- Identifying hospitalized patients who meet hospice eligibility criteria and to arrange educational meetings with patient and their families to discuss hospice services and as appropriate schedule admissions to UH prior to discharge
Contributes to patient and family counseling and education
Participates in ongoing interdisciplinary comprehensive assessment
Assesses patient / family psychosocial response to the terminal illness and evaluates coping skills
Assists patients, family, and agency staff in utilizing community resources
Assists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family
Coordinates with team members to plan and implement the hospice plan of care
Social Worker will participate in interdisciplinary team meetings
Social Worker maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following UH documentation standards
Participates in and is actively involved with the bereavement portion of the program as appropriate
Incorporates organization’s commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency
