Due to increasing volumes, United Hospital District has 2 Home Health/Hospice RN positions available serving primarily Faribault and Martin Counties! Responsible for holistic skilled nursing care on an intermittent basis to clients in their places of residence. Ensures continuity of care through case management to include performance of skilled assessments, interventions, teaching, maintenance, patient advocacy and interacting with community/agency resources to provide an ongoing safe environment. Must have current RN license in Minnesota and be able to obtain Iowa RN license after hire. Approximately 32 hours per week benefits eligible position. This position does include taking call.
Salary : $36 - $54
