Schedule: Monday - Friday | 8a - 4:30pm | No weekend or Holidays & flexible with scheduling.

Overview
As a Speech-Language Pathologist (SLP) in our PACE (Program of All-Inclusive Care for the Elderly) model of care, you will play a vital role in delivering comprehensive speech, language, and swallowing therapy services for older adults. Your expertise will directly impact on the quality of life for our participants by addressing their unique needs through personalized care plans.
What you’ll do
- Interdisciplinary Team Collaboration: Work closely with physicians, nurses, therapists, social workers, dietitians and other care staff to ensure comprehensive participant care, as needed to ensure holistic care.
- Assessment & Care Planning: Perform initial and periodic assessments, developing personalized care plans to address speech, language, and swallowing needs.
- Records Management: Maintain up-to-date speech-language records, document evaluations, treatment plans, and progress per organizational standards.
- Service Delivery: Provide SLP services across the Holland and Muskegon areas, including participant homes and contracted placements, ensuring alignment with PACE care goals.
- Education & Support: Train participants, families, and staff on swallowing safety and communication strategies.
- Communication & Collaboration: Engage in team meetings and case conferences to share insights and collaborate with families and the IDT to optimize care.
- Clinical Competence: Apply evidence-based approaches in speech-language pathology, especially for the geriatric population.
- Bilingual (Spanish/English) is a plus.
What it takes
- Master’s degree in Speech-Language Pathology from an accredited program.
- Certification of Clinical Competence (CCC) from ASHA.
- Current Michigan professional license (or completion of Clinical Fellowship Year for eligible candidates).
- Reliable transportation and valid Michigan driver’s license.
- Current CPR certification (or willingness to obtain).
- Strong interpersonal and communication skills.
- Ability to adapt treatments to cultural and linguistic needs.
- Commitment to person-centered care with strong organizational skills.
- Experience with elderly, long-term care, or end-of-life care preferred.
