What are the responsibilities and job description for the Post Foster Care Support Program Case Manager position at United Neighborhood Centers NEPA?
The Post Foster Care Support Program Case Manager provides supportive services to homeless youth & families who have aged out of the foster care system, assists to find suitable housing, and provides life skills support to participants. Additionally, they provide administrative support by completing weekly hotel authorizations for OYFS involved families. A strong candidate must possess comfort with excel, data entry, customer service in the form of in person and electronic correspondence.
Responsibilities:
- Assist in the intake process for transitioning youth based & coordinate referrals from Children & Youth Services
- Contract with program participants in setting goals that work toward self-sufficiency.
- Provide client transportation, as needed.
- Assist with job coaching, budgeting, and education goals.
- Assist clients with activities of daily living including cooking, cleaning, laundry and personal hygiene.
- Provide follow-up services for program participants once they’ve graduated from the program.
- Recruit and educate landlords about the Post Foster Care Program, and the benefits of participation.
- Help participants to read and understand lease agreements.
- Conduct intake processes, assessing the needs of individuals and families who walk in for our services.
- Maintain accurate client files, financial documentation, and record keeping, including entering comprehensive client information into the ClientTrack database.
- Link participants with various supports with the school districts or childcare options.
- Conduct home and office visits with consumers.
- Assist in reporting and documentation of program development and achievements to meet obligations of funders.
- Coordinate efforts with other UNC programs, including referrals.
- Attend SLCoC Youth & Education bi-monthly meetings.
- Complete weekly hotel authorizations, as assigned by OYFS and UNC Leadership/
- Enter service and program information into ClientTrack client management system.
- Any other duties deemed necessary by Community Services Director.

- Experience working with individuals from diverse backgrounds.
- Computer literate with knowledge of MS Office and other common applications.
- Ability to work independently and as a team member.
Education/Experience/Licensure
- High School diploma or equivalent is required.
- Bachelor degree in social work, counseling, or related field is preferred.
- Strong interpersonal skills, ability to work with diverse populations and good communication skills are required.
