Overview
We are seeking a dedicated and compassionate Client Care Coordinator to join our team. The Care Coordinator is responsible for developing and managing the client's care of plan and communicating with clients, families, social workers etc. The Care Coordinator plays an integral role in interfacing with clients and families to ensure safe, appropriate, and meaningful care is provided. We are looking for someone who has a positive demeanor at all times and is able to effectively multitask in a high functioning office environment.
Responsibilities
- Run Assessments
- Conduct comprehensive assessments to identify client needs and develop individualized care plans.
- Supervise Caregivers, providing guidance on best practices in care management.
- Maintain accurate documentation of client interactions, progress notes, and treatment plans in compliance with regulatory standards.
- Manages schedules for all patients
- Effectively communicates care plan with clients, families and caregivers
- Conducts client quality calls and provides follow-up as needed to family members, caregivers and others.
- Fill in for shifts if you don't have a backup caregiver
Qualifications
- Basic office and computer skills and organizational abilities
- Familiarity with care plans
- Strong organizational and communication skills
- Ability to manage patient interactions with empathy and professionalism
- Detail-oriented with a focus on accuracy in documentation and scheduling
Job Type: Full-time
Pay: $23.00 per hour
Expected hours: 40 per week
