Community Living Counselor – Milner House Group Home
Location: Plainfield, CT
Seeking Community Living Counselors
We are looking for dedicated, hard-working professionals to work in our licensed Transitional Living Program in Moosup, Connecticut. Working with our adults in mental health recovery to return to living independently. Work as part of a team providing individual support and group counseling in a residential setting. Milner House is a 14-bed intensively staffed transitional living facility. It serves people with severe and prolonged mental illness. The facility provides 24-hour coverage on three awake-shifts and is licensed by the Connecticut Department of Public Health. Work includes assisting and teaching individuals in adult daily living skills and social and emotional skill development. A valid driver’s license, reliable transportation, and the ability to transport clients in their own car are required.
Responsibilities include:
- Medication monitoring
- Transporting individuals that we serve
- Assists individuals in addressing medical and psychiatric needs
- Prepares and facilitates educational and skill building groups and one-to-one interventions as directed by the treatment plan
- Assist in the preparation of meals
- Individual and group counseling
- Crisis intervention and risk assessment
- Function as part of a team in support of available services
- Coordinate care with providers and families as needed
- Documentation
- House cleaning
Comprehensive training provided for all positions.
Schedule:
- Sunday - Monday: 7:00 am - 3:30 pm (16 - hours) 1st Shift
- Tuesday - Saturday: 12:30 pm - 9:00 pm (40 - hours) 2nd Shift
Community Living Counselor Requirements:
· 1st and 2nd shift position – Associate’s Degree plus two years of appropriate experience required or Bachelor’s Degree in relevant human services field.
· 3rd shift position – High School Diploma or equivalent plus four years of appropriate experience required.
United Services, Inc. requires all employees to be vaccinated against influenza annually.
Community Living Counselor Salary Range: $18.54 - $20.98 per hour depending on experience
Bilingual Spanish/English Fluency: $19.48 - $22.04 per hour depending on experience.
