Shift: Days
Duties of the RN Case Manager
- Discharge Planning & Referrals: Coordinate patient discharges to home, community, or SNFs. Manage housing, dialysis, wound vac, and shelter needs.
- Caseload Management: Handle 17-25 patients per shift, collaborating with Social Workers.
- Interdisciplinary Teamwork: Participate in rounds with physicians and team members.
- Independent Work: Work autonomously while coordinating with staff.
- Time Management & Problem Solving: Prioritize tasks and think critically to resolve challenges.
- EMR Documentation: Use Allscripts to manage patient records.
- Preferred Knowledge: Familiarity with CarePort, MCG, and/or InterQual.
- Patient Units: Focus on Med-Surg and Tele units, covering inpatient care.
Requirements needed for the RN Case Manager
- Current Registered Nurse (RN) license in New York State.
- Proven experience in patient care within a hospital or acute care setting.
- BLS (Basic Life Support)
- PRI (Patient Review Instrument) Certification
