Position Summary:
The Oncology Navigator is responsible for the navigation and optimization of the experience of Prostate Cancer patients as they move through the continuum of cancer care from screening and detection to diagnosis, treatment, and survivorship. The Oncology Navigator is to assist the physician and staff in ensuring the delivery of superior patient experience.
Primary Duties & Responsibilities:
- Act as a single point of contact for prostate cancer patients, their families, and the Urologist.
- Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs and communicate any concerns to the Urologist.
- Assist the patient and office staff in scheduling tests, procedures, appointments, and treatments.
- Help develop an appropriate treatment plan in consultation with the Urologist that is understandable and feasible to the patient.
- Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on a multidisciplinary care plan.
- Assist patients in overcoming barriers to care (transportation, financial counseling).
- Make sure to coordinate care between patients and the Urologist, Radiation Oncologist, and all other care providers- both inside and outside the organization.
- Refer patients to supportive care services such as dieticians, social workers, and financial counselors.
- After consultation with the Urologist, help eligible patients access appropriate clinical trials.
- After consultation with the Urologist, educate patients about appropriate new technologies.
- Direct patients and families to available community resources and supportive services.
- Coordinate discharge planning with the urologist and office staff.
- Participate in multidisciplinary consultations/conferences.
- Document recommendations made through consultations/conferences.
- Participate in prostate cancer disease planning and help ensure compliance.
- Assist in the development of patient education materials, clinical pathways, and quality studies.
- Work with the Urologist, Radiation Oncologist, and office staff to ensure timely appointments, results reporting, and communication of patient care and follow-up.
- Promote community education and increase prevention awareness and early detection screening programs.
- Monitor and report on outcomes measures to Comprehensive Prostate Cancer Program Director, Prostate Cancer Patient Navigator, and Program Administrator.

Qualifications
Qualifications:
- High school diploma required, Bachelor’s degree is a healthcare-related field preferred.
- Completion of a Medical Assistant program or one year of Medical Assistant experience in a medical clinic is required. Urology experience is a plus. (ALL REGIONS EXCEPT ARIZONA).
- Graduated from an accredited Medical Assistant program required and will be verified prior to employment; Medical Assistant Certification preferred. (ARIZONA ONLY).
- Clinical experience working in a medical or healthcare setting is required.
- Extensive knowledge of healthcare.
- Urology experience preferred but not essential.

- Proficient knowledge of medical terminology.
- Strong communication skills, both verbal & written.
- Proficient computer software and database skills.
- Comfortable working in a fast-paced environment.

- Maintain HIPAA compliance.
- Multitasking and proactive problem-solving.

Physical Requirements for the Job:
- Regularly required to sit and stand for extended periods.
