This role involves coordinating all aspects of patient care, including scheduling appointments, managing treatment plans, and serving as the primary point of contact between the patient, healthcare providers, and external resources. The Treatment Coordinator plays a critical role in enhancing the patient experience, ensuring timely access to care, and facilitating seamless communication across the care team.
Primary Duties & Responsibilities
- Serve as the primary liaison between patients, families, and the healthcare team, providing clear communication and updates on treatment plans.
- Coordinate all aspects of patient care, including scheduling consultations, treatments, follow-up appointments, and any necessary diagnostic tests.
- Collaborate with physicians, nurses, and other clinical staff to ensure that each patient’s treatment plan is implemented accurately and efficiently.
- Provide education and resources to patients and families about prostate cancer treatment options, side effects, and self-care practices.
- Monitor patient progress, identify any barriers to care, and work to resolve issues promptly.
- Maintain detailed and accurate patient records, documenting all interactions, treatment plans, and changes in care.
- Facilitate access to support services such as financial counseling, social work, and community resources.
- Ensure compliance with all clinical protocols, regulations, and company policies.
- Participate in multidisciplinary team meetings to discuss patient cases and treatment strategies.
- Continuously seek opportunities to improve patient satisfaction and outcomes through feedback and process improvement initiatives.
Qualifications
Qualifications:
- Bachelor’s degree in healthcare administration, nursing, or a related field; a clinical background (RN, LPN) is preferred.
- Minimum of two to three (2-3) years of experience in a healthcare coordination role, preferably within oncology or urology.
- Familiarity with prostate cancer treatments, including surgery, radiation, hormone therapy, and emerging therapies.
- Strong knowledge of medical terminology and healthcare procedures.
- Experience working with electronic health records (EHR) systems.
- Exceptional communication and interpersonal skills, with the ability to build trust and rapport with patients and their families.
- Highly organized with strong attention to detail and the ability to manage multiple tasks simultaneously.
- Empathy and a patient-focused approach to care.
- Problem-solving skills with the ability to handle complex situations with professionalism and discretion.
- Proficiency in Microsoft Office Suite and EHR systems.
- Ability to work independently and collaboratively within a multidisciplinary team.
- A strong commitment to maintaining patient confidentiality and following HIPAA guidelines.
