What are the responsibilities and job description for the Permanent and Transitional Housing Case Worker position at UNITED VETERANS BEACON HOUSE, INC?
PURPOSE
Provide direct and indirect services to individuals, groups, and communities with the objective of achieving mutual client system and worker goals. Improve the quality of life, enhance social functioning, and empower program participants and their families. Aide in contributing to building long-term sustainability within the lives of our program participants enrolled in Permanent Housing and the VA’s Grant and Per Diem Program. Connect program participants with the services and resources necessary to help achieve and maintain their goal of independent living.
REQUIREMENTS
· 2 years of case management experience or bachelor’s degree in Mental Health Counseling or Human Services related field or a minimum.
· Experience, preferably in a residential program serving the homeless or similar population.
· Experience working with Substance Abuse and Mental Illness preferred.
· Respectful and considerate of the population being served.
· Experience using data entry platform, HMIS, preferred.
· Familiarity with overall philosophy, purpose, and mission of United Veterans Beacon House.
ESSENTIAL JOB FUNCTIONS
· Attend weekly case conferences with Senior Case Worker.
· Responsible for the weekly maintenance and general up-keep of UVBH vehicles.
· An assessment is to be completed to identify the individual needs of the resident within 30 days, diagnosis to be identified.
· Conduct weekly walk-throughs of assigned UVBH residences.
· Conduct monthly fire drills with the house manager and clients of assigned UVBH residences.
· Conduct monthly site inspections.
· Attend weekly house meetings at assigned UVBH residences.
· An independent living plan will be developed and implemented within the first two weeks of admission into our program. The I.L.P. is intended to track progress and evaluate outcomes.
· Make referrals and provide resources to address the individual needs of the residents. Explore the residents’ eligibility for referrals or information; this should be given high priority.
· Provides periodic reports to supervisor on the progress of each case including noticeable changes in appearance and behavior.
· Follow-up with maintenance for all repairs and replacements.
· Provide weekly In’s & Out’s reports to Senior Case Worker.
· Update rents charge form as needed.
· Review monthly print out and submit corrections to Senior Case Worker within 5 business days.
· Review annual inspections and weekly walk-through reports to address any corrective actions.
· Performs record keeping functions required by the agency, e.g., case records to meet state standards, HMIS, and the Current Client list.
· Assist residents who are inappropriately referred to relocate within forty-eight hours.
· Identifies residents in need of external services and contacts other social services agencies. Provides paperwork as needed regarding the residents’ situation.
· Performs annual inspection of each home.
· Attends interdisciplinary team meetings with outside agencies.
· Attend agency or professional seminars and conferences to increase professional knowledge.
· Rotating on-call weekends with other Case Workers.
· Maintaining a minimum of two face-to-face meetings per month with each client (GPD).

• Workers are representatives of United Veterans Beacon House and are, therefore, expected to conduct themselves in a way that reflects accountability to the agency, the community, and the client system.
• Does not abuse sick time or personal days.
• Consistently reports to work on time and is ready to begin work at the start of the shift.
• Always provides proper notification when late or absent.
• Performs on-call duties as outlined in On-Call Policy.
• Available to work overtime when necessary.
• Demonstrates the ability to recognize priorities in the organization of workflow.
• Able to perform duties independently, with minimal need of direct supervision.
• Maintains the confidentiality of clients, shelter/programs, agency and or United Veterans Beacon House.
• Reports to work well-groomed and in compliance with dress code.
• Wears ID Badge while on duty.
• Demonstrates the ability to interact in a positive and helpful manner with clients, visitors, contract staff, volunteers, and co-workers.
• Reflects commitment to building a supportive work environment and maintains a positive attitude at the workplace and towards job.
• Projects a good image in dealing with the public clients.
• Is willing to make an extra effort to help build a quality and caring shelter/program.
• Employees may be required to carry out other duties as assigned.
