Position Overview:
UWNYC’s Health Equity strategy is a preventative access care initiative designed to increase access to wellness, primary care and screening services for poor and low-income populations who have been historically underserved. BIPOC communities continue to be disproportionately affected by chronic disease, cancer, and poor infant and maternal health, and pandemics such as the COVID-19 epidemic in the United States. In response to these racial health disparities, the UWNYC became the lead implementation partner for the Choose Healthy Life (CHL) initiative, which was launched in 2021, in five cities (Atlanta, Detroit, Newark, New York City, District of Columbia) to promote and conduct COVID – 19 testing and vaccination, leveraging networks in the Black-faith community to lead implementation and build confidence in health interventions. CHL-NYC currently funds 20 NYC churches and is now in its third year of New York State Department of Health funding and expanding to upstate New York. The upstate expansion will add 10 new churches and partner with other United Ways to implement program activities, expand emphasis on health promotion to prevent chronic diseases, increase access to care among BIPOC residents, and implement the blueprint for Wellness screening, a Quest Diagnostics test that provides vital health information to empower individuals about their disease risks.

The UWNYC is seeking to hire a Health Navigator Supervising Director for the upstate expansion of Choose Healthy Life to 10 new churches. The Health Navigator Supervising Director will travel to Syracuse, Albany, Newburgh, Rochester and Buffalo every other month. They will meet with church partners every week and will support lay health workers in implementing the Choose Healthy Life program which is a health promotion and screening program. They will be the liaison between upstate United Way programs ensuring that the churches receive the resource support to implement services and will be the primary liaison for the NYC Clergy Leader.

Primary Responsibilities:
- On-boarding and engagement of 10 churches / upstate pastors and training of Health navigators.

The oversight and management of navigator and church activities related to the CHL program including:
- Overseeing and supporting directing outreach and community engagement with support from the NYC clergy leader and broader ministry.
- Timely health events to reach at-risk individuals in the community and within the church congregation.
- Coordinate with health navigators at the 10 churches wellness & health events.
- Coordinating and attending regular training to develop navigation expertise and other community engagement and communication skills needed for the role and understanding community resources and services.
- Overseeing the implementation of the NYSDOH funded program activities and program compliance.
- Ensure that the church Health navigators are trained in CHL client care coordination skills and will ensure that Health navigators follow up with clients that need help in accessing health related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, proper use of an emergency room, overcoming barriers to obtaining needed medical care and /or social services.
- Set up and run weekly check-in Health Navigator meetings/calls with churches and the NYC -based Health Navigator Supervisor to share insights, troubleshoot issues, and expand knowledge.
- Operate in a compliant manner related to handling of patient health information (PHI).
- Regularly assessing the needs, preferences, and other ongoing efforts of the church congregation to tailor outreach and messaging and seize new opportunities as they arise for advancing CHL’s.
- Documents reports of CHL outreach and community engagement activities, client encounters and contacts made on behalf of clients; completes and submits monthly CHL wellness event client coordination reports; documents clients screening, and outcomes achieved by client in an effective manner.
- Reports to the VP for Health Equity to ensure program adherence and alignment with the strategic goals for the UWNYC Health Equity Unit and elevates issues and gathers input for planning and program implementation.
- Supports the NYC Clergy leader to plan monthly planning meetings and engagement with clergy and community on program and questions.
- Supports the navigators in implementing their health promotion and blueprint for Wellness programs and action plans and ensures that they meet program goals and objectives.

Creating a schedule of wellness and health events, including church-led events and/or
- Providing culturally appropriate health education and information.
- Community care coordination: Closing the loop to care and social determinates of health resources (i.e. can be done by phone etc.)
Skills, Knowledge, and Experience:
- Bachelor of Science in health, social work or public policy.
- Minimum of 5 years of supervisor experience.
- Minimum of 5 years of program and contract management.
- Strong communications, writing and community outreach and organizing skills.
- Ability to document activities, share progress, elevate issues, and communicate with supervisors in a timely fashion.
- Proficiency with technology and Microsoft Office (Word and Excel) and Power Point.
- Experience in managing contracts and grants management.
- Comfort and familiarity building new relationships with community organizations whose services are valuable to community members.
- Experience planning and executing events, including logistical planning, outreach, and mobilization.
- Record of community leadership or involvement in church events, especially through a health ministry.
- Prior experience in community engagement or organizing.
- Interest in public health topics and training.

Salary Range:

$70,327 - $ 112,522*
- The final determination of salary, within this range, will depend on the candidate’s education, skills and experience required for the particular position.

- Successful completion of a background check.
- UWNYC is presently on a Hybrid/In-Person work schedule.
