JOB SUMMARY
UWSA seeks a Care Navigator, Opportunity Youth, responsible for assisting in overall project outreach, recruitment, intake and support for youth and young adults between the ages of 12 and 24 years old for the Tomorrow's Promise Initiative. This is a two-year grant-funded position that will expand United Way's work in this area and ensure compliance with all programmatic and investment management aspects that address youth disconnection. The Care Coordinator will support the Director, Opportunity Youth Initiatives and the Impact Manager, Opportunity Youth in staffing practitioner meetings linking youth and young adults to information and community resources related to reconnecting to either education or career pathways. This person will have the ability to work on a mobile work modality and communicate with a diverse set of stakeholders in a professional manner. The successful candidate will be flexible, self-motivated and able to work both independently and collaboratively.
PRIMARY DUTIES AND RESPONSBILITIES
- Conduct home visits and other outreach efforts to reengage K-12 students in school, juvenile justice, and other supports using the school district provided leaver lists.
- Conduct system navigation through intake, screening, assessment, referral, and connection to reengagement providers.
- Support young people and their families by providing information and education on these topics, making appropriate referrals to services both internally and externally while providing continued support, advocacy and follow-up as needed.
- Maintain a list of local programs, resources and services that can serve youth and / or their families.
- Maintain communication with community providers regarding service availability and accessibility.
- Networks, actively engages, and collaborates with other Community Impact Areas, Dual Generation Initiative, Family Resource Center, 211, Help Me Grow, UP Partnership, City of San Antonio, and school districts.
- Contribute to the implementation and monitoring of an outreach plan that encompasses all recruitment and retention strategies for at-risk youth.
- Conduct aggressive outreach initiatives by attending and participating in events and meetings to promote Tomorrow's Promise Initiative and generate community awareness for opportunity youth prevention, intervention, and diversion strategies.
- Provide advocacy and follow-up services as necessary to youth participants.
- Respect and maintain confidentiality; follow all agency and program guidelines regarding confidentiality.
- Perform other duties needed to help drive the work of Successful Students Impact Council.
