What are the responsibilities and job description for the Community Navigator position at United Way of the Crossroads?
Summary
The Community Navigator is a full-time position that serves as a social service liaison to residents of Victoria, Goliad, DeWitt, and Lavaca Counties, and the City of Gonzales. The Community Navigator will meet with individuals in need, conduct intake assessments to determine their needs, and educate clients about available resources. They will assist clients in developing a plan forward and advise them on their next steps. Additionally, the Community Navigator will work to establish and strengthen community support systems and networks.
The Community Navigator will collaborate closely with the Community Health Worker (CHW) team based in Victoria to establish operational protocols and ensure effective data collection.
Responsibilities
- · Assist community members by assessing barriers and providing appropriate service referrals.
- · Support individuals by offering referrals, follow-up contacts, and collaborating with community partners as needed.
- · Act as a liaison between the Community Navigator program and other external partners.
- · Maintain accurate daily logs, interaction notes, and files for all community members.
- · Collect and enter client data into the case management system.
- · Educate clients to enhance their ability to advocate for necessary resources in their communities.
- · Attend staff meetings, community health worker meetings, and relevant training sessions.
- · Perform other duties as assigned.
Requirements:
- · High school diploma or equivalent, with at least 2 years of experience working in underserved communities, preferably in community health/outreach, healthcare settings, or emergency/disaster case management.
- · Certification as a Community Health Worker (CHW) or willingness to obtain CHW certification upon hire.
- · Bilingual (English/Spanish) skills preferred.
- · Access to a personal vehicle, a valid Texas Driver's License, car insurance, and a clean driving record. Flexibility to travel to off-site locations within or outside the Crossroads area as needed.
- · Detail-oriented with the ability to manage multiple projects and tasks independently, with minimal supervision.
- · Proficiency in Microsoft Office (Word, Excel, Outlook) and online resources/applications; familiarity with database software.
- · Strong interpersonal skills, with the ability to communicate effectively with community professionals, nonprofit leaders, and corporate executives in diverse settings.
- · Ability to cope with change, adapt quickly, and handle risk and uncertainty with composure.
- · Team player, flexible, and capable of working with humor and grace under pressure.
- · Ability to occasionally lift and/or move up to 25 pounds.

- · Frequently exchanges information in person and/or in writing via telephone and computer.
- · Regularly required to sit, stand, walk, reach with hands and arms, talk and hear.

- · Occasionally positions self to reach files, binders, etc. above the head and/or near the floor.
- · Occasionally must lift and/or move up to 25 pounds.
