Position Overview:
We are seeking a dedicated Patient Advocate to join our team. In this role, you will work closely with patients, providers, and SNF staff to assist our patients in getting the care they need.
Key Responsibilities:
- Coordinate with patients, providers, and SNF staff to ensure all appropriate consents are collected
- Provide support and education to patients about their healthcare options.
- Coordinate transitions for patients entering and exiting wound care plans, facilitating communication with healthcare providers.
- Develop and maintain strong relationships with patients to ensure their needs are met effectively.
- Advocate for patients' rights and assist them in understanding their benefits and entitlements.
- Collaborate with interdisciplinary teams to ensure holistic care for patients.
- Stay updated on changes in healthcare policies and programs to provide accurate information to patients.
Qualifications:
- Bachelor’s degree in a related field (e.g., healthcare, human services). Social work background is preferred
- Experience working in a healthcare setting or in a patient support role is preferred.
