Healthcare Team Lead Position

Job Summary: We're seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a CareCoach Connect provider. In this role, you'll work closely with patients, families, and healthcare teams to deliver high-quality care and improve patient outcomes.

Key Responsibilities:

• Provide care in patient's homes and virtually consistent with their licensure and/or certification
• Detect and manage acute and chronic conditions, including diabetes, hypertension, kidney disease, and other medical conditions
• Order and interpret diagnostic lab work, x-rays, and EKG
• Prescribe medications and non-pharmacological interventions
• Collaborate with healthcare teams to develop patient-centered plans of care
