Position in this function provides leadership in the coordination of patient centered care across the continuum, develops a safe discharge plan through collaboration with patients and caregivers and multidisciplinary healthcare team. Facilitates episode of care to maximize appropriate level and LOS, patient experience and quality of care., and level of care., and reimbursement for all patients.

Primary Responsibilities:

  • Drives to appropriate DRG length of stay, level of care and patient experience, and reimbursement level of care for all patients
  • Promotes patient advocacy in an independent care model
  • Develops an optimal, l patient individualized, transition plan for patients with goal to reduce readmission and to support continued plan of care, regardless of next level of care 
  • Maintains a working knowledge of current care management practices and philosophy; including changes within, care coordination changes, utilization review changes, authorization changes, contracts changes, regulatory requirements, etc.; and serves as an educational resource to all Health System staff regarding utilization review and discharge planning 
  • Screens patients for admission, and concurrent medical necessity, and post-acute care needs
  • Assessment of physical, psychosocial, & economic needs for transition of care planning to a variety of levels of care; delegates to others as appropriate. Assesses patients who have been identified as needing post needing post-acute transition or care management needs

Required Qualifications:

  • Associates Degree
  • RN License in the State of Maine, current, unencumbered, or compact state licenses
  • BLS certification with American Heart Association
  • 1 years of experience of nursing experience 

Preferred Qualifications:

  • Demonstrated ability to work in a multi-disciplinary team
  • Demonstrated ability to identifies solutions to non-standard requests and problems
  • Demonstrated ability to work autonomously and as a high performer 
  • Demonstrated ability to solve moderately complex problems and/or conducts moderately complex analyses

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
