Job Description
We are seeking a skilled NP or PA to join our HouseCalls team in Bladen and Columbus Counties, NC. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes. This critical preventive care helps identify and reduce health risks for patients, as well as coordinating appropriate follow-up care to improve health and well-being.
Required Skills and Qualifications
To be successful in this role, you must possess an active NP or PA license, with the ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions. Additionally, you should have Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification (for NPs) or Physician Assistant national certification through NCCPA (for PAs). You must also be able to complete the physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessments. Reliable transportation is also essential for completing home visit assessments.

You will not be required to work on-call, weekends, or holidays.
