">The Role: As a member of our Home and Community Care team, you'll provide annual clinical assessments to patients in their homes, helping identify and reduce health risks. This important preventive care also coordinates appropriate follow-up care to improve health and well-being.
">Your Responsibilities: Conduct home visits, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams. Identify diagnoses to inform care management and active medical management. Communicate findings with members' primary care physicians and intervene in urgent or emergent situations.
">We're Looking For: Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions. NPs: Must have approval to practice autonomously or without formal supervision from their licensing board, if applicable. New hires must apply to practice autonomously within 1 month of hire. PAs: Ability to work under a Collaborative Agreement in applicable states.
