Job Description
The Per Assessment NP HouseCalls role involves conducting annual clinical assessments in patients' homes, identifying and reducing health risks, and coordinating follow-up care to improve health and well-being. This requires strong clinical skills, attention to detail, and excellent communication abilities.
Key Responsibilities:
- Conduct in-home assessments, including past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
- Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
- Communicate findings in your assessment that will be used to inform members' PCP of potential gaps in care.
- Develop and implement individualized care plans to address patients' needs.
