The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.

Schedule: Monday - Friday, 8:00AM - 5:00PM

Location: 245 Terracina Suite #202, Redlands, CA 92373

Primary Responsibilities:

  • Support patients over the phone and in-person to assist in completing forms (Registration and immigration forms; CalOptima SHA forms, scheduling appointments, facilitating referral appointments, access to resources and understanding their plan of care
  • Collaborates with the health plan and network care coordination staff to ensure timely communication of patients’ clinical information
  • Guide patients in understanding and accessing the benefits they are entitled to under Medicare and Medi-Cal
  • Assist with coordination of patients’ health care and social service needs both within and outside of the medical group and CalOptima
  • Facilitate communication of initial care plan (ICP) to the primary care physician (PCP), CalOptima, patient and other members of the ICT
  • Assist enrollee in meeting preventive care goals
  • Assist enrollee with scheduling of appointments
  • Initiate referrals to both internal and external care management departments and other department/government or community agencies
  • Facilitate warm transfers to patient’s assigned case manager in accordance with patient needs, when appropriate
  • Notify patient’s care team of care concerns
  • Maintain accurate records and documentation
  • Effectively interview patients to determine the strengths, problems, functional status, goals and need for specific services and resources
  • Develop rapport, establish and maintain effective interpersonal relationships with internal and external contacts at all levels
  • Work with network case management staff to expedite the resolution of patient concerns
  • Remain knowledgeable of patients’ benefit structure
  • Communicate effectively, both verbally and in writing with individuals from varying cultural and ethnic backgrounds
  • Regular and consistent attendance
  • Complies with all policies and procedures, including but not limited to HIPAA and Fraud, Waste and Abuse
  • Other duties and responsibilities, not specifically described, maybe defined or assigned from time to time, consistent with knowledge, skills and abilities of the incumbent by management

Required Qualifications:

  • High School Diploma/GED (or higher) 
  • 1 years of customer service experience with strong communication skills both in person and via phone
  • Must have a current and valid California driver’s license

Preferred Qualifications:

  • Previous experience with medical insurance
  • Prior experience with EMR computer applications
  • Working knowledge of general office procedures 

   

The salary range for this role is $16.88 to $33.22 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
