As a Travel Nursing Opportunity in Home Health, you will be responsible for providing high-quality patient care in a home health setting. You will also work closely with our multidisciplinary team to ensure seamless care coordination and effective communication with patients, families, and other healthcare providers.
Responsibilities
- Assess patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions to address these needs
- Develop and implement individualized care plans, taking into account patients' goals, preferences, and medical histories
- Deliver evidence-based interventions to promote optimal health outcomes, including wound care, medication management, and education on disease self-management
- Collaborate with our interdisciplinary team to ensure comprehensive care planning and effective communication with patients, families, and other healthcare providers
- Document patient interactions, progress, and outcomes in accordance with regulatory requirements and internal policies
Requirements
- Current licensure as an RN in the state where you will be practicing
- BSN degree or higher from an accredited nursing program
- 1 year of experience in a home health or similar setting
- Ability to lift up to 25 pounds and navigate stairs, if necessary
- Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues
- Proficiency in electronic health records (EHRs) and other relevant software applications
