POSITION SUMMARY:

The Discharge Planner Case Manager will work with attending physicians, specialists and treatment team to coordinate the patient's transition and discharge following in patient care.

ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.

  • Performs all aspects of discharge planning for assigned caseload according to the hospital's written policies and procedures
  • Identifies the strengths, needs, abilities, and preferences of each person served
  • Assists in implementing the individual plan of each person served
  • Secures services that are consistent with the needs of each person served through direct interaction with that person and/or with individuals identified by that person
  • Makes initial contact with referral sources and potential patients
  • Assist with coordination of the referral and intake process
  • Determines appropriate level of care, as well as appropriateness of available services
  • Responsible for assisting with maintaining admission reports or screening logs; to include a review of all available information, as well as discussions with referral sources
  • Assist with maintaining a waiting list with continual review and updating of the list, to include how admissions will be prioritized
  • Assist with ensuring procedures for referral to necessary care, including medical and crisis care, are developed for persons on the waiting list
  • Makes recommendations for alternative services or disposition to the referral source, when a person is found ineligible for services and maintains documentation of these actions
  • Ensures that each person served, and/or legal representative receives an appropriate orientation to the facility and types of services provided upon admission
  • Organization of patient charts prior to discharge
  • Reviews patient records and prioritizes clinical issues
  • Documents discharge arrangements, when patient/family relay this information
  • Ensures all referrals are documented
  • Assist with completing admission paperwork
  • Initiates patient satisfaction surveys
  • Complete discharge packets to ensure patient has pertinent plan of care information
  • Engages patients and responsible parties in the discharge process
  • Completes documentation that is pertinent to discharge transition
  • Completes and documents a descriptive arrangement of treatments and other community resources for the provision of follow-up services, i.e. dates, names, address, and service providers
  • Complete and document a plan for medical/physical treatment
  • Documents the condition of the patient at the time of discharge
  • Completes psychosocial assessments, developments treatment planning, and modalities, and ensure those correlates with discharge planning goals as deemed appropriate
  • Assist with post-discharge follow-up to reduce readmissions
  • Reviews community education follow-up for assessing continued service needs
  • Supervises the Admission Coordinators duties when related to discharge planning
  • Offer discharge planning services to the patient in the way of supportive, individual, family, or group therapy focused on discharge goals of the patient. Carry out a liaison role with community resource providers
  • Offer supervisory assistance with group modalities as deemed appropriate
  • Adheres and respects all policies and procedures regarding anonymity and confidentiality of all patient records past and present. This covers any written or verbal communications regarding patient's identify, address, and situation.
  • Functions in a friendly, supportive, courteous, respectful, cooperative, and professional manner with patients, families, physicians, referral sources
  • Other duties as assigned by the Supervisor

Required Skills:

  • Communication skills and active listening

Required Work Experience:

  • Two (2) Years of clinical experience in a psychiatric setting
  • Medical terminology is essential to this position

Licensing/Certification/Education Requirements:

  • RN/LPN with psychiatric nursing experience preferred: or a degree in health-related field from an accredited school.
  • Current State of Tennessee License RN or LPN
  • Current CPR and CPI Certification required
  • Current State of Tennessee Driver’s license
  • Current Auto liability Insurance according to Company Policy

Other Requirements:

  • Position may require unscheduled overtime, week-end work
  • Must be willing and able to work flexible hours
  • Travel required

Preferred
  • Technical/other training or better
  • Associates or better in Nursing
