1. Education : Graduate of an accredited college or university with at least a bachelor’s degree in social work. Holds a current Arkansas license as a social worker.
2. Training and Experience : One to three years experience in social work field.
3. Job Knowledge : Knowledge of psychosocial needs of families and individuals of all age groups, including neonates and infants, toddlers, children, adolescents, adults and geriatric populations, related to the impact of illness, hospital admission, diagnosis, treatment, discharge and post-hospital care. Knowledge of community organizations and / or resources. Knowledge of medical terminology and related medical social work knowledge desirable.

DESCRIPTION :
Responsible for assessment and identification of need for the provision of social work, discharge planning and case management services to the organization’s customers for the purpose of ensuring positive outcomes for customers’ health, safety and well-being. Responsible, jointly with other case management team members, physicians and staff for ongoing post-hospital care planning which is timely and appropriate and which increases customer satisfaction, reduces unnecessary length of inpatient hospitalization and effectively provides for continued quality care. Responsible for the management of the activities of the Social Work Services Department. Accountable for insuring the quality of services rendered within the Social Work Services Department’s scope of services, and accordingly, the planning development and implementation of departmental policies, procedures, programs, services, goals and objectives in keeping with the organization’s philosophy and culture. This position requires knowledge in the care of patients served as indicated : Infant-adolescent is 0 to 17 years, Adult is 18 years to 65 years and Geriatric is 65 years and above.
