Social Worker (MSW) - Contract Position
Location: Silver Spring, MD, 20910
Duration: 3 months, Full-Time (Onsite)
Schedule: Monday - Friday, 8:00 AM - 4:30 PM (40 hours per week)
Pay Rate: $45.86/hr
Job Overview
We are seeking a compassionate and dedicated Social Worker (MSW) to join our Care Management team at a well-established healthcare facility in Silver Spring, MD. This is a contract role requiring expertise in hospital discharge planning, social care plans, and resident assessments. The ideal candidate will possess a Maryland state license, Basic Life Support (BLS) certification, and a Master of Social Work (MSW) with 1-3 years of experience in discharge planning.
Key Responsibilities
- Participate in discharge planning and development/implementation of social care plans.
- Perform administrative duties, such as completing medical forms, reports, and arranging transportation.
- Act as a liaison with other departments to ensure coordinated patient care.
- Facilitate interdisciplinary team meetings to promote patient well-being and appropriate discharge planning.
- Provide ongoing communication with patients, families, and responsible persons regarding care status and discharge.
- Offer guidance on Medicare/Medicaid and other financial assistance programs.
- Maintain HIPAA compliance and patient confidentiality at all times.
- Support quality initiatives in Skilled Nursing Units (SNU) and Acute Rehabilitation Units (ARU).
- Track and address grievances related to social services.
- Conduct assessments and deliver services based on patient-specific needs, ensuring safety and care standards are met.
Minimum Qualifications
- Education: Master of Social Work (MSW) required. LCSW preferred.
- Experience: 1-3 years of hospital discharge planning experience within the past 3 years.
- Licenses/Certifications:
- Maryland State License (Required to interview).
- Basic Life Support (BLS) Certification.
- Skills: Strong autonomy, professional judgment, and reliability in handling psychosocial care, discharge planning, and patient advocacy.
