Job Overview

Duties
- Assist patients with personal hygiene, grooming, and bathing, etc.
- Monitor and observe patient conditions, reporting any changes to healthcare professionals.
- Help with meal preparation and ensure proper nutrition.
- Provide medication reminders following HIPAA regulations.
- Maintain accurate medical records and documentation of patient care.
- Provide companionship and emotional support to patients.
- Assist with mobility and lifting patients safely as needed.
- Support long-term care plans tailored to individual patient needs.
- Engage in home community care activities that promote patient engagement.
Experience
Candidates should possess the following skills and experience:
- Knowledge of medical terminology and HIPAA compliance is preferred.
- Experience in long-term care or home health settings is a plus.
- Proficiency in patient observation techniques to identify changes in health status.
- Home Health Aide certification, active and unencumbered.
- Ability to prepare nutritious meals tailored to dietary restrictions.
- Experience with maintaining medical records accurately.
- Strong interpersonal skills to build rapport with patients and families.
