Admissions Registered Nurse
The Admissions Registered Nurse is responsible for making an initial visit to the patient and family to explain and initiate services. The primary responsibility of this nurse is to complete a comprehensive assessment of the patient, environment, and support system and admit to the appropriate program. The admissions nurse will complete an initial plan of treatment and necessary forms to implement patient care. He/she will work collaboratively with the interdisciplinary team to meet the immediate needs of the patient and family.
Territory: This position is based in Green Bay, with travel primarily to Green Bay and Appleton areas, as well as occasional travel outside of the territory as needed.
Schedule: Working 4 days per week, 11:00 am - 8:00 pm - 36 hours per week, every 4th weekend work, call back up 10-12 times per year, 1-2 holidays per year.
Key Job Duties:
- Review information concerning the patient’s medical background
- Identify the appropriate decision maker/agent and follow through with Durable Power of Attorney for Healthcare (DPOAHC), activation and “In Lieu Of” forms
- Provide an explanation of hospice and palliative programs and answer questions appropriately
- Review and complete admission forms with the patient/family
- Complete a comprehensive nursing assessment which includes medication reconciliation and obtain orders as needed
- Collaborate with members of the medical team to determine appropriate services as needed
- Notify referral sources and attending physicians of admission to the hospice or palliative program
- Assess need for and order supplies and durable medical equipment
- Enter appropriate information into the electronic medical record and initiate follow up
- Communicate with the primary team and support services to meet the needs of the patient
- Collaborate with the Director of Patient Services and other departments as appropriate on process improvement efforts
- Participate in focus groups or other specialized work groups
- Shares in providing 24-hour, seven day-a-week coverage

Requirements:
1. Associate degree in Nursing required, BSN preferred
2. Current unrestricted registered nurse licensure from the State of Wisconsin
3. Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage
