What are the responsibilities and job description for the Hospice Chaplain position at Unity Hospice?

We are looking for an outstanding Chaplain to join our team in Madison (Dane County), WI. and the surrounding area.

Core Responsibilities:
- Conduct spiritual assessments of incoming patients and manage a caseload
- Participate in developing a plan of care to meet patient and family spiritual needs
- Serve as liaison and coordinate care with community religious resources
- Participate in an on-call rotation for evenings and weekends
- Provide grief and loss support
Day to Day Activities:
- Manage a case load of patients by creating your own visit schedule
- Conduct daily visits where patient resides
- Collaborate with fellow Hospice team members
- Deliver outstanding care
- Foster relationships with your patients and families to support them in finding meaning and purpose in their greatest time of need.
Who the Job reports to:
- Director of Clinical Services
Requirements/ Qualifications:
- Ordination, commissioned or credentialed according to the practices of an organized religious group
- Masters of Divinity/Theology or equivalent.
- BCC Board Certified or Board Certifiable
- Completed 4 units of Clinical Pastoral Education or currently completing 4th unit.
- 1 year experience in a healthcare setting
- Driver’s license, insurance, and vehicle
