Qualifications:
High School diploma and five years experience or Associates Degree in human services/related field and three years experience in relevant Human Services field required. OPWDD Broker training completion is required within first 90-days of employment. Computer skills and keyboarding familiarity required. Ability to be flexible and work in a team environment. Excellent interpersonal, communication, problem solving, customer service and leadership skills essential. Evidence of understanding and empathy for the PWDD population. Previous knowledge/experience with Person Centered Planning, advanced person centered planning, developing a person centered budget, self-advocacy/self-development, planning team meeting/circle of Support, writing/providing service plans, OPWDD regulations, Medicaid regulations and budgets preferred. Demonstrated ability and knowledge to work professionally with multiple services provider agencies, etc., required. Valid NYS Driver's License required and acceptable per organization policy, ability to travel.
Full time, 40 hours per week. Monday - Friday 8am-4pm.
