">Primary Responsibilities:
- Evaluate patient goals and develop discharge plans that minimize readmissions.

Minimum Requirements:
- Registered Nursing degree from an accredited institution.
- Valid RN licensure in the state of practice.
- One year of experience in geriatric or mental health settings preferred.
- Excellent communication and interpersonal skills.
