Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy to help them navigate complex systems. Responsibilities
- Assess client needs and develop personalized care plans that address their unique circumstances.
- Provide crisis intervention and support to clients facing immediate challenges.
- Collaborate with healthcare providers, social services, and community organizations to coordinate services.
- Monitor client progress and adjust care plans as necessary to ensure effective outcomes.
- Offer mentoring and support to clients in areas such as behavioral health, childcare, and workforce development.
- Maintain accurate records of client interactions, progress notes, and service referrals.
- Treat members with respect while providing guidance and support in their lives.
- May assume advocate role on the member’s behalf to ensure approval of the necessary supplies/services for the patient in a timely fashion.
- Manage own maximum caseload of 75, while providing monthly care to all members as needed and reducing barriers to care.
- Work in cooperation with co-workers and supervisory staff.
Experience
- A background in social work or a related field is preferred.
- Experience in crisis management and intervention techniques is essential.
- Familiarity with Medical processes and regulations is a plus.
- Previous experience in healthcare or behavioral health settings is advantageous.
- Demonstrated ability to mentor individuals or groups effectively.
- Knowledge of child welfare policies and workforce development initiatives is beneficial.
- Strong communication skills and the ability to work collaboratively with diverse populations are required.
Education: College, GED, or Highschool diploma equivalent.
Experience: 1 year or equivalent to an in-clinic service delivery of care or managed care environment in the capacity of a care coordination role.
Other Requirements: Possession of valid driver’s license and proof of state required auto liability insurance. Reliable transportation.
