UnityPoint Clinic is searching for an Anticoagulation Clinic with General Cardiology Nurse Practitioner to join a well-established Cardiology practice in Cedar Rapids, Iowa.

The UnityPoint Health – Cedar Rapids Heart & Vascular Institute encompasses the heart care specialists and services of St. Luke’s Hospital, St. Luke’s Cardiology and the cardiovascular surgeons from Physicians’ Clinic of Iowa. This collaborative relationship allows us to provide the highest quality of care with fewer complications, better outcomes and higher survival rates.

  • Established Cardiology practice with 15 Physicians and 8 Advanced Practice Providers who provide coverage at UnityPoint Health - St. Luke's Hospital.
  • Main office is located in Cedar Rapids, with satellite offices in Manchester and Anamosa, plus servicing 3 other outreach communities.
  • Diverse practice covers sub-specialization in interventional cardiology, structural heart disease, peripheral vascular, electrophysiology and heart failure.
  • Part of a large multi-specialty group, including primary care, providing a built-in referral base.
  • Utilizes EPIC as an electronic health record both for inpatient and outpatient services.

Responsibilities

The Anticoagulation ARNP will provide and manage care to patients enrolled in the UnityPoint Cardiology Anticoagulation Therapy (CAT) Specialty Clinic.

  • Knowledge of coagulation and thrombogenesis.
  • Accurate assessment of medical problems and history, medication review, and social determinates of health , along with shared decision making to prescribe appropriate anticoagulant medication for each patient
  • Assess bleeding and clotting risk utilizing bleeding risk score tools to facilitate anticoagulation management plan during planned interruption of anticoagulation therapy.
  • Serve as the liaison for inpatient information for hospital providers and ambulatory providers.
  • Initiate, discontinue, or adjust anti-thrombotic therapy in accordance with the plan of care.
  • Follow all aspects of the patient’s anti-thrombotic therapy.
  • Assess appropriate drug information and literature resources and provide effective information to varied audiences including patients and other health care professionals, and peers.
  • Education of new patients. Effective patient teaching regarding anticoagulation therapy and maintaining recommended INR check schedule, if on warfarin and guidelines for direct oral anticoagulants.
  • Assessment of patient ability to understand anticoagulation therapy and follow dosage instructions.
  • Assess patient needs and provide ongoing education regarding medical concerns, medical procedures, nutrition and maintenance of health and wellness.

Qualifications
  • Nurse Practitioner
  • Board Certified
  • 2 years of cardiology experience preferred
  • Certificate of completion of on-line Anticoagulation Therapy Management within 6 months from date of hire
  • Hold an unrestricted medical license in the state of Iowa
  • ACLS required
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
  • Eligible to work in the United States
