Overview
Under the guidance and supervision of the Regional Director Care Management provides a broad range of support services to assist licensed social workers and licensed care coordinators with transition planning across the care continuum. Assists all team members of the Integrated Care Management team in discharge planning activities to support the timely transition of patients to the next site of care.

Responsibilities
Key Accountability –Care Coordination · Collaborates with Integrated Care Management team members with discharge planning activities. · Clearly communicates with team members and key stakeholders in a timely manner to support timely discharge from the acute level of care. · Prioritizes and completes assignments independently and in a timely manner. Able to meet urgent deadlines successfully. · Arranges for patient transport to the next site of care. Includes but not limited to wheelchair vans, ambulance and other modes of transport as needed. · Schedules post discharge follow up visits with the patient’s PCP. May arrange follow up visits with specialty physicians. · Organize and schedule family meetings at the request of social work or care coordination · Follow up on cases with Medicaid pending on a weekly basis and update leadership and team members in a timely manner. · Research and track Medicare days for patients with frequent admissions and long length of stay. · Obtains authorization information for admissions to Skilled Nursing Facilities or Inpatient Rehab Facilities · Completes on-line E-filing updates with the Iowa Courts as needed · Facilitates FMLA paperwork completion to support patients and families. · Performs Notary Public duties as needed. · Provides support for the assignment and follow up on Milliman Care Guidelines (MCG) when assigned · Supports Behavioral Health Bed Placement as needed · Performs scheduling for the UnityPoint Health Des Moines Wheelchair van when the primary scheduler is out of the office. · Distributes notification letters, provides education, answers questions and secures signatures from patients per CMS regulations surrounding admission status and the right to appeal discharge. |

Basic UPH Performance Criteria · Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. · Demonstrates ability to meet business needs of department with regular, reliable attendance. · Employee maintains current licenses and/or certifications required for the position. · Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare. · Completes all annual education and competency requirements within the calendar year. · Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
