Responsibilities
- Lead and support clinical staff through direct supervision, coaching, performance management, and competency evaluations to ensure high-quality, compliant care in the home care and/or hospice setting.
- Coordinate and oversee patient care delivery by facilitating interdisciplinary communication, ensuring effective care planning and referrals, and addressing patient or caregiver concerns to support positive clinical and satisfaction outcomes.
- Serve as a subject matter expert on regulatory standards, documentation requirements, and electronic health records, ensuring compliance with state, federal, and payer regulations.
Education (One of the Following):
- Graduate of State Board approved program for Registered Nurses
- Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association
- Master’s degree in speech-language pathology or Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOT
- Two years home care and/or hospice case management experience.
- Valid driver’s license when driving any vehicle for work-related reasons.
- Current license to practice nursing or therapy according to state where service is provided.
- CPR Certification
- Valid Mandatory Reporter course completion by state requirement.
