Overview
Abbe Center for Community Mental Health is seeking a full time Nurse for our Integrated Health Home (IHH) program in our Manchester office. Nurse Care Manager’s work with a team of professionals to enhance the physical and mental well-being of clients in the IHH program. Emphasis is on understanding client’s health needs and promoting wellness/ health education activities.

Responsibilities
Direct contacts with clients to develop personalized health goals, provide health education along with planning, promoting and implementing wellness and prevention activities.
Collecting and tracking health status measures and data through the use of Medicaid websites and electronic health records.
Coordination of care with IHH team members including medical and mental health professionals.
This position includes Integrated Health Home hours that are office-based and some work in client’s homes in Manchester and surrounding area (Delaware, Buchanan and Jones County)
Qualifications
Requires a valid Iowa Nursing License and a minimum RN level.
Mental Health experience preferred.
Valid Iowa driver’s license.
